After cleaning and sanitizing, equipment and utensils: Shall be air-dried or used after adequate draining before contact with food.
Insects, rodents, and animals not present — Inspector Note: Observed multiple drain flies at facility in multiple areas. The presence of insects, rodents, and other pests shall be controlled to minimize their presence on the premises by: Routinely inspecting the premises for evidence of pests.
Insects, rodents, and animals not present — Inspector Note: Observed visible light entry at back door of main kitchen. Outer openings of a food service establishment shall be protected against the entry of insects and rodents by: Solid, self-closing, tight-fitting doors.
Food-contact surfaces: cleaned & sanitized — Inspector Note: Observed drink dispenser at bar area with residue not clean to sigh and touch. Equipment food-contact surfaces and utensils shall be clean to sight and touch.
Food-contact surfaces: cleaned & sanitized — Inspector Note: Observed food contact surface of interior of multiple microwaves in need of cleaning. Equipment food-contact surfaces and utensils shall be cleaned: At a frequency specified by the manufacturer; or absent manufacturer specifications, at a frequency necessary to preclude accumulation of soil or mold.
Proper hot holding temperatures — Inspector Note: Observed fried chicken at warmer in prep area tempering below 135F for less than 4hours. Observed chicken patty improperly hot-holding below 135 at main kitchen. Hot and Cold Holding. Except during preparation, cooking, or cooling, or when time is used as the public health control, time/temperature control for safety food shall be maintained at 135°F (57°C) or above. COS: PIC voluntarily discarded chicken patty and PIC reheated fried chicken at fryer.
Plumbing installed; proper backflow devices — Inspector Note: Observed leak at piping at the handwashing sink near the dish machine. Advised PIC that a plumbing system shall be repaired according to law and maintained in good repair. Advised PIC to have handwash sink repaired.
Adequate ventilation and lighting; designated areas used — Inspector Note: Observed coats stored on top of boxes of soda dispensers at the dry storage area. Advised PIC that Lockers or other suitable facilities shall be located in a designated room or area where contamination of food, equipment, utensils, linens and single-service and single-use articles cannot occur. Advised PIC that all personal belongings would need to be relocated to the designated area.
PIC present, demonstrates knowledge, performs duties — Inspector Note: Observed allergy awareness training without 9 major food allergens listed (missing sesame). Advised PIC that employees shall be properly trained in food safety, including food allergy awareness, as it relates to their assigned duties. Food allergy awareness includes describing foods identified as major food allergens and the symptoms that a major food allergen could cause in a sensitive individual who has an allergic reaction. Advised PIC to include all 9 major food allergens and to retrain employees.
Food-contact surfaces: cleaned & sanitized — Inspector Note: Observed food contact surface of interior of the bulk ice machine stored as clean with black mold-like substance. Advised PIC that equipment food-contact surfaces and utensils shall be clean at any time during the operation when contamination may have occurred. COS: PIC had food employee empty bulk ice machine and the interior.
