Food and nonfood-contact surfaces cleanable, properly designed, constructed, and used — Inspector Note: Observed cold well in the main kitchen with an ambient temperature of 51.8 degrees F. Equipment shall be maintained in a state of repair and condition that meets the requirements specified in subsections (1) and (2) of this Rule. Stated to the PIC to have unit serviced.
Insects, rodents, and animals not present — Inspector Note: Observed excessive fly activity throughout the establishment. The presence of insects, rodents, and other pests shall be controlled to minimize their presence on the premises by using methods, if pests are found, such as trapping devices or other means to control pests. Stated to PIC to contact pest control.
Certified Food Protection Manager — Inspector Note: Observed CFSM certificate not posted in public view. The original CFSM certificate shall be posted in public view in each food service establishment. Stated to PIC to post the CFSM certificate in public view.
Adequate handwashing facilities supplied & accessible — Inspector Note: Observed the front food service hand sink not reaching a minimum temperature of 85 degrees F. A handwashing sink shall be equipped to provide tempered water of a temperature of at least 85 degrees F. Stated to the PIC to have the front-food service hand sink serviced.
Adequate handwashing facilities supplied & accessible — Inspector Note: Observed the front food service hand sink stored with ice. Also observed the main kitchen's hand sinks storing a chemical bottle inside. A handwashing sink shall be maintained so that it is accessible at all times for employee used and may not be used for purposes other than hand washing. COS: PIC removed ice and chemical bottle from hand sinks.
Insects, rodents, and animals not present — Inspector Note: Upon arrival, observed back door of facility propped open, when not in use.. Advised PIC except as specified in paragraphs 2 through 5 of this subsection, outer openings of a food service establishment shall be protected against the entry of insects and rodents by: solid, self-closing, tight-fitting doors. COS: PIC closed back door of facility.
PIC present, demonstrates knowledge, performs duties — Inspector Note: Observed no employee food allergen awareness training document onsite. Advised PIC employees are properly trained in food safety, including food allergy awareness, as it relates to their assigned duties. Food allergy awareness includes describing foods identified as major food allergens and the symptoms that a major food allergen could cause in a sensitive individual who has an allergic reaction.
Certified Food Protection Manager — Inspector Note: Observed CFSM not posted. Advised PIC the original CFSM certificate shall be posted in public view in each food service establishment. An additional copy shall be retained on file at the food service establishment at all times, and shall be made available for inspection by the Health Authority.
Hands clean and properly washed — Inspector Note: Observed employee adjust garments and continue prepping without hand wash after contaminating. Observed employee handle cell phone and then proceed to prep fries at fryer basket. Advised PIC food employees shall clean their hands and exposed portions of their arms immediately before engaging in food preparation including working with exposed food, clean equipment and utensils, and unwrapped single-service and single-use articles and: after touching bare human body parts other than clean hands and clean, exposed arms: after engaging in other activities that contaminate the hands. COS: Had employees cease, wash hands, and don new gloves.
