CA: Nonfood contact surfaces shall be kept free of an accumulation of dust, dirt, food residue, and other debris. Keep areas clean to the sight and touch. Instructed to clean shelves.
Insects, rodents, and animals not present — Inspector Note: Observed minor visible light entry at back door of kitchen area leading to the outside. Advised PIC outer openings shall be protected to prevent the entry of pest by tight fitting doors, screens, or other methods. CA: Discussed with PIC to obtain weather-stripping.
Insects, rodents, and animals not present — Inspector Note: Observed roaches in kitchen and in men's restroom. Pest control report was reviewed and observed that establishment was treated this morning at 9:45 7-20-26. Suggested to PIC to follow recommendations report left by pest control company.
Physical facilities installed, maintained, and clean — Inspector Note: Observed excessive build up on floors along base of walls in kitchen and under grills, wok and fryers. Items shall be free of accumulations.
