The permit holder shall notify consumers by written notification of the presence of major food allergens as an ingredient in unpackaged food items that are served or sold to the consumer. Stated to PIC to place written notification statement regarding food allergens to the public.

Food-contact surfaces: cleaned & sanitized — Inspector Note: Observed mechanical dish machine being used sanitizing equipment in the main kitchen with 0 ppm CL. A chemical sanitizer used t=in a sanitizing solution for mechanical operation at contact times specified under subsection (8)(b)3 of this Rule shall meet the requirements specified in DPH Rule 511-6-1-.07(6)(g). COS: Had food employee prime dish machine to gain adequate sanitization concentration levels of 100 ppm CL.

Contamination prevented during food preparation, storage, display — Inspector Note: Observed dripping on top boxes of avocado, oranges, and beans in Walk-In-Cooler from fan unit. Advised PIC Food shall be protected from contamination that may result from a factor or source not specified under paragraphs (a) through (y) of this subsection. COS: Had PIC relocate boxes away from dripping.
