Employees are properly training in safety, including food allergy awareness, as it relates to their assigned duties. Stated to PIC to obtain a food allergy awareness training and thoroughly train employees in food allergies.
Management knowledge, responsibilities, reporting — Inspector Note: Observed PIC could not demonstrate in a verifiable manner that food employees are/were aware of their responsibilities to inform regarding foodborne illnesses and symptoms. An employee health agreement form(s) shall be produced upon request of the Health Authority. Stated to PIC to have employee health agreement forms readily available upon request of the Health Authority.
Food-contact surfaces: cleaned & sanitized — Inspector Note: Observed pink mold like substance on interior white panel of ice machine in need of cleaning. Equipment food-contact surfaces and utensils shall be cleaned: At a frequency specified by the manufacturer; or absent manufacturer specifications, at a frequency necessary to preclude accumulation of soil or mold.
Proper hot holding temperatures — Inspector Note: Observed multiple TCS food items improperly hot holding at front warmer cabinet. Also observed plantains improperly hot holding at the front food service steam table 1. TCS foods shall properly hot hold at 135 degrees F. or above. Stated to PIC to reheat all items at 165 for proper reheating for hot holding.
Food and nonfood-contact surfaces cleanable, properly designed, constructed, and used — Inspector Note: Observed the WIC with an ambient temperature of 47.1 degrees F. Equipment shall be maintained in a state of good repair as specified under subsections (1) and (2) of this Rule. Stated to PIC to have unit serviced.

Hands clean and properly washed — Inspector Note: Observed multiple food employees wash hands at the designated hand sinks in the main kitchen and front-food service area with cold water. Discussed with PIC and food employees proper cleaning procedures: rinsing hands under clean, running warm water. COS: Had food employees rewash hands at the designated hand sinks with clean, running warm water.

Response procedures for vomiting & diarrheal events — Inspector Note: Observed facility without an EPA-approved disinfectant in order to respond to vomit/fecal events. Procedures and EPA-approved disinfectant shall be readily available on-site in order to respond to vomit/fecal events. Stated to the PIC to obtain EPA approved disinfectant.
Proper cold holding temperatures — Inspector Note: Observed multiple TCS food items (shredded cheese, raw chicken quarters, cut cabbage, and raw chicken wings) cold holding in the temperature danger zone above 41 degrees F. in the main kitchen's WIC. TCS food shall properly cold hold at 41 degrees F. or below. COS: PIC discarded shredded cheese and cut cut cabbage and relocated raw chicken wings and raw chicken quarters for proper cooling.
Adequate ventilation and lighting; designated areas used — Inspector Note: Upon arrival, observed backpack stored on top of bulk-food item containers. Advised PIC lockers or other suitable facilities shall be provided and used for the orderly storage of employees' clothing and other possessions. COS: Had PIC relocate employee backpack.
PIC present, demonstrates knowledge, performs duties — Inspector Note: Observed no food allergy awareness training at time of inspection. Advised PIC employees are properly trained in food safety, including food allergy awareness, as it relates to their assigned duties. Food allergy awareness includes describing foods identified as major food allergens and the symptoms that a major food allergen could cause in a sensitive individual who has an allergic reaction.
Proper hot holding temperatures — Inspector Note: Observed container of cooked oxtail and collard greens hot-holding in warmer unit, that was turned off at time of inspection below 135F for less than 4 hours, observed one container of jerk chicken hot-holding for less than 4 hours below 135F at front-food service steam table #2. Advised PIC Hot and Cold Holding. Except during preparation, cooking, or cooling, or when time is used as the public health control, time/temperature control for safety food shall be maintained at 135°F (57°C) or above. COS: PIC turned on and increased heat on warmer unit to initiate reheating procedure, also advise to speed up reheat on stove top.
