Physical facilities installed, maintained, and clean — Inspector Note: Observed heavy staining on ceiling tiles in the main kitchen. CA:The physical facilities shall be cleaned as often as necessary to keep them clean and by methods that prevent contamination of food products.
Adequate ventilation and lighting; designated areas used — Inspector Note: Observed low light in the main kitchen. CA: Light intensity shall be adequate to prevent contamination in food facility.
Response procedures for vomiting & diarrheal events — Inspector Note: Observed facility with clean up procedures but no supplies for clean up. CA: A food establishment shall have written procedures for employees to follow and an EPA registered disinfectant effective against Norovirus available on-site at the food service establishment when responding to vomiting or diarrheal events that involve the discharge of vomitus or fecal matter onto surfaces in the food service establishment.
Food-contact surfaces: cleaned & sanitized — Inspector Note: Observed cleaning of utensils and prep containers not being sanitized in the main kitchen. CA:Manual Warewashing Equipment, Chemical Sanitization Using Detergent-Sanitizers. If a detergent-sanitizer is used to sanitize in a cleaning and sanitizing procedure where there is no distinct water rinse between the washing and sanitizing steps, the agent applied in the sanitizing step shall be the same detergent-sanitizer that is used in the washing step.
Food-contact surfaces: cleaned & sanitized — Inspector Note: Observed excessive debris on the interior of the microwave in main kitchen. CA:Equipment food-contact surfaces and utensils shall be clean to sight and touch.
Nonfood-contact surfaces clean — Inspector Note: Observed nonfood-contact surfaces of lids on the storage rack w/ encrusted debris stored w/ clean items. Advised PIC non-food contact surfaces shall be cleaned to prevent accumulation.
Physical facilities installed, maintained, and clean — Inspector Note: Observed heavy staining of ceiling tiles in the main kitchen area. Also observed large holes in walls throughout kitchen area. Advised physical facilities shall be maintained in good condition at all times.

Food stored covered — Inspector Note: Observed food uncovered on counter and reach in cooler. Advised PIC all foods shall be protected at all times. COS: PIC covered items.
