Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris.
PIC present, demonstrates knowledge, performs duties — Inspector Note: Observed no food allergen awareness training for employees. Also, Observed no key-drop contract on file. Employees are properly trained in food safety, including food allergy awareness, as it relates to their assigned duties. Food allergy awareness includes describing foods identified as major food allergens and the symptoms that a major food allergen could cause in a sensitive individual who has an allergic reaction. Employees are visibly observing and verifying delivered foods as they are received to determine that they are from approved sources and are placed into appropriate storage locations, as required by this Chapter, such that they are received and maintained at the required temperatures, protected from contamination, unadulterated, and accurately presented, by routinely monitoring the employees’ observations, maintaining receiving/corrective action records for deliveries during non-operating hours and periodically evaluating foods upon their receipt as specified within DPH Rule 511-6-1-.04(3)(m).
Certified Food Protection Manager — Inspector Note: Upon Arrival, observed no CFSM posted. The original CFSM certificate shall be posted in public view in each food service establishment. An additional copy shall be retained on file at the food service establishment at all times, and shall be made available for inspection by the Health Authority. COS: Had PIC post CFSM in public view.
Food-contact surfaces: cleaned & sanitized — Inspector Note: Observed food contact surface of multiple lids stored as clean on storage rack with stains. Equipment food-contact surfaces and utensils shall be clean to sight and touch.
Nonfood-contact surfaces clean — Inspector Note: Observed nonfood contact surfaces of multiple shelving racks in the walk-in cooler in the main kitchen stored as clean with debris and accumulation of residue. Also observed excessive food debris at the bottom of the reach in freezer in the main kitchen. Advised PIC nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris. Advised PIC to thoroughly clean shelving racks in the walk-in cooler and the interior of the reach-in freezer.

Employees are visibly observing and verifying delivered foods as they are received to determine that they are from approved sources and are placed into appropriate storage locations, as required by this Chapter, such that they are received and maintained at the required temperatures, protected from contamination, unadulterated, and accurately presented, by routinely monitoring the employees’ observations, maintaining receiving/corrective action records for deliveries during non-operating hours and periodically evaluating foods upon their receipt as specified within DPH Rule 511-6-1-.04(3)(m). Advised PIC to obtain Key Drop Delivery Contract and to maintain contract on file.
Management knowledge, responsibilities, reporting — Inspector Note: PIC could not demonstrate in a verifiable manner that food employees are/were informed of their responsibility to report symptoms and illnesses related to foodborne diseases. COS: PIC provided employee health policy to the Health Authority.
Food-contact surfaces: cleaned & sanitized — Inspector Note: Observed food contact surfaces of multiple pizza pans with excessive carbon buildup and debris. Advised PIC that all food contact surfaces shall be cleaned to sight and touch. Advised PIC to wash, rinse, and sanitize after each use and to discard pizza pans that have excessive carbon buildup.
