Proper cooling methods used: adequate equipment for temperature control — Inspector Note: Observed cooked macaroni and raw chicken cooling tightly covered in walk-in-cooler. Advised PIC Cooling shall be accomplished in accordance with the time and temperature criteria specified under DPH Rule 511-6-1-.04(6)(d) by using one or more of the following methods depending on the type of food being cooled: Loosely covered, or uncovered if protected from overhead contamination during the cooling period to facilitate heat transfer from the surface of the food.

Physical facilities installed, maintained, and clean — Inspector Note: Observed stains on ceiling tiles in main kitchen area. Advised PIC the physical facilities shall be cleaned as often as necessary to keep them clean and by methods that prevent contamination of food products.
Food stored covered — Inspector Note: Observed one metal container of cut tomatoes stored uncovered at bottom portion of prep-top cooler and two containers of flour stored uncovered when not in use, near fryer basket and grill. Advised PIC except as specified under paragraph 2. of this subsection and when cooling as specified in DPH Rule 511-6-1-.04(6)(e)2.(ii), storing the food in packages, covered containers, or wrappings, except for loosely covered or uncovered containers in which food is being cooled if protected from overhead contamination.
Food-contact surfaces: cleaned & sanitized — Inspector Note: Observed microwave in need of cleaning food debris food contact surface. Advised PIC equipment food-contact surfaces and utensils shall be cleaned: At a frequency specified by the manufacturer.
Food properly labeled; original container — Inspector Note: Observed multiple sauce bottles not labeled, observed multiple juices prepped on site without label inside front-service display cooler. Advised PIC that foods shall be labeled with name of contents/ common name.
Proper cooling methods used: adequate equipment for temperature control — Inspector Note: Observed chicken eggroll and steak eggroll cooling in disrepaired RIC #1; observed containers of cooked chicken wings and cooked chicken cooling tightly covered in the walk-in cooler; observed cooked chicken wings cooling at room temperature on counter near fryer basket; observed shredded cheese and sour cream cooling tightly covered in RIC #2 at the back of the kitchen. Advised PIC cooling shall be under refrigeration loosely covered. COS: PIC relocated chicken and steak eggroll, covered shredded cheese, sour cream, and relocated wings cooling on counter.

Food and nonfood-contact surfaces cleanable, properly designed, constructed, and used — Inspector Note: Observed cooler drawer with an ambient temperature of 59 degrees F. Also observed RIC #1with an ambient temperature of 50 degrees F. Advised PIC that all equipment shall have an ambient air temperature that is maintained in good repair and be accurate within the intended range of used.
PIC present, demonstrates knowledge, performs duties — Inspector Note: Observed no food allergy awareness training, Advised PIC employees shall be trained on food allergen awareness.
Adequate handwashing facilities supplied & accessible — Inspector Note: Observed multiple containers stored at the back hand sink. Advised PIC hand sinks shall be used for handwashing purposes only.
Adequate handwashing facilities supplied & accessible — Inspector Note: Observed no soap at hand sink in the back area of the kitchen. Advised PIC soap shall be stored at the hand sink at all times.
Proper cold holding temperatures — Inspector Note: Observed multiple items in cooler drawer (raw shrimp and raw chicken) cold holding in the temperature danger zone holding above 41F. Advised that when cold holding TCS food items, foods shall range between 41 degrees F. or below. COS: PIC discarded raw shrimp and raw chicken from cooler drawer.
