Equipment shall be in a state of repair according to subsections (1) and (2) of this Rules stated to PIC to have unit repaired.

The physical facilities shall be cleaned as often as necessary to keep them clean by methods that prevent contamination of food products. Stated to the PIC to thoroughly clean ceiling tiles in the main kitchen.

Stated to PIC to have current employees sign employee health agreement forms and produce to the Health Authority upon request.
