Physical facilities installed, maintained, and clean — Inspector Note: Observed severe dust accumulation on ceiling tiles throughout the kitchen area. Advised PIC that the physical facility shall be cleaned as often as necessary to keep them clean and by methods that prevent contamination of food products. Advised PIC to thoroughly clean ceiling tiles.

Adequate ventilation and lighting; designated areas used — Inspector Note: Observed multiple disposable water bottles stored on prep tables holding clean utensils in the main kitchen area. Advised PIC areas designated for employees to eat drink, and us tobacco products and electronic devices shall be located so that food, equipment, linens, and single-use/single-service articles are protected from contamination. Advised PI to relocate disposblae water bottles to a designated area.

Insects, rodents, and animals not present — Inspector Note: Observed visible light entry at the back door of the facility. Advised PIC that all outer openings shall be protected from pests and insects by solid, self-closing, tight-fitting doors. Advised PIC to place a sealant on the back door of the facility.
