Physical facilities installed, maintained, and clean — Inspector Note: Observed buildup on floors along base of walls in kitchen, dish room and in dry stock areas. Observed damaged multiple floor tiles in walk in cooler. Floors and tiles shall be maintained in good condition and free of accumulation.
