The permit holder shall notify consumers by written notification of the presence major food allergens as an ingredient in unpackaged food items that are sold or served to the consumer. Stated to PIC to obtain written notification regarding food allergens and to post in public view.

Adequate handwashing facilities supplied & accessible — Inspector Note: Observed hand sink 1 in the main kitchen with water temperature of less than 85 degrees F. (83F). A handwashing sink shall be equipped to provide a tempered water at a temperature of at least 85 degrees F. Stated to PIC to have hand sink serviced.

Food-contact surfaces: cleaned & sanitized — Inspector Note: Observed ceiling-food contact surface of the interior of microwave stored as clean with excessive food debris and residue. Equipment food contact surfaces and utensils shall be cleaned at any time during the operation when contamination may have occurred.

Proper cold holding temperatures — Inspector Note: Observed multiple TCS foods (bean sprout, and multiple raw shell eggs) cold holding in the temperature danger zone above 41 degrees F at the prep top cooler 2 in the main kitchen. Also observed a container of white rice cold holding in the temperature danger zone above 41 degrees F in the main kitchen. TCS foods shall cold hold at 41 degrees F. or below. COS: PIC placed raw shell eggs, bean sprouts, and white rice in the WIC for cooling.

Posted: Permit/Inspection/Choking Poster/Handwashing — Inspector Note: Observed the 2026 permit posted in public view upon arrival. Advised PIC that a current Food Service Establishment permit shall be posted in public view at all times. Advised PIC to repost the 2025 Food Service Establishment permit in public view.

Warewashing facilities: installed, maintained, used; test strips — Inspector Note: Observed a food employee wash hands at the 3-compartment sink in the main kitchen. Advised PIC a warewashing sink may not be used for handwashing. COS: Had food employee cease washing hands at the 3-compartment sink and directed the employee to wash hands at the designated hand wash sink.

Advised PIC that Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris. Advised PIC to thoroughly clean the interior of RIF 2.

Food stored covered — Inspector Note: Upon arrival, observed multiple containers of seasonings and sauces near the prep area by the cookline stored uncovered when not in use. Advised PIC storing the food in packages, covered containers, or wrappings, except for loosely covered or uncovered containers in which food is being cooled if protected from overhead contamination. Advised PIC to place covers on multiple seasonings and sauces when not in use.
