Advised PIC to post the most recent inspection report in public view.

Advised PIC to post the permit in a location in the food service establishment that is conspicuous to consumers. Advised Pic to obtain a permanent 2026 Food Service Establishment permit and post in public view.
Food and nonfood-contact surfaces cleanable, properly designed, constructed, and used — Inspector Note: Observed cardboard lining shelving above crock pot. Advised PIC nonfood-contact surfaces of equipment that are exposed to splash, spillage, or other food soiling or that require frequent cleaning shall be constructed of a corrosion-resistant, nonabsorbent, and smooth material. COS: PIC removed cardboard lining.
Management knowledge, responsibilities, reporting — Inspector Note: Observed PIC could not demonstrate in a verifiable manner that food employees are/were aware of their responsibility to report symptoms and illnesses of foodborne diseases. Advised PIC that an employee health agreement form(s) shall be produced upon request of the Health Authority. Advised PIC to obtain signed copies of employee health agreement forms.

Single-use/single-service articles: properly stored, used — Inspector Note: Observed multiple single-use articles (plastic containers for tzatziki sauce) ised to store other food products (celery, carrots, eggroll, and spring roll) in multiple areas of the kitchen including the prep table and walk in cooler. Advised PIC single use articles may not be reused.

Physical facilities installed, maintained, and clean — Inspector Note: Observed cardboard on floor near fryer baskets at the prep area. Advised PIC materials for indoor floors shall be smooth, durable, and easily cleanable.
Adequate ventilation and lighting; designated areas used — Inspector Note: Observed personal cups stored on rack above the prep top cooler in the main kitchen. Advised PIC locaters shall be provided and used for storage of employees' possessions. COS: Had PIC relocate cups.

Advised PIC employees shall consume food only in approved designated area separate from food preparation and serving areas. COS: Had employee cease eating at prep area and wash hand at the hand sink.
Adequate handwashing facilities supplied & accessible — Inspector Note: Observed paper towels not supplied at the designated hand sink in the main kitchen. Advised PIC that paper towels shall be supplied at all hand sinks. COS: Food employee supplied hand sink in the main kitchen with paper towels.
Food stored covered — Inspector Note: Observed multiple foods in the WIC (Philly eggroll, cooked chicken wings, broccoli, onions, and spring rolls) stored uncovered and not in use. Also observed at the prep table in the main kitchen a container of celery and carrots stored uncovered. Advised PIC that except when cooling, foods shall be stored covered in order to protect from contamination. Advised PIC to place covers on all food products

Advised PIC except for containers holding food that can be readily and unmistakably recognized, such as dry pasta, working containers holding food or food ingredients that are removed from their original packages for use in the food establishment shall be clearly and legibly identified, in English, with the common name of the food. Advised PIC to label all sauce bottles with common name in English.
Approved thawing methods used — Inspector Note: Upon arrival, observed raw chicken wings thawing in stagnant water at the meat prep sink in the main kitchen. Advised PIC on proper thawing methods: completely submerged under running water at a water temperature of 70°F (21°C) or below. Advised PIC to turn on running water at 70 degrees F. or below at the meat prep sink for proper thawing.

Advised PIC nonfood-contact surfaces of equipment that are exposed to splash, spillage, or other food soiling or that require frequent cleaning shall be constructed of a corrosion-resistant, nonabsorbent, and smooth material. Advised PIC to remove cardboard from shelving units and replace with durable, easily cleanable, and nonabsorbent material.

Response procedures for vomiting & diarrheal events — Inspector Note: Observed facility without approved EPA registered disinfectant in order to respond to vomit/fecal events. Advised PIC that a food establishment shall have written procedures for employees to follow, and an EPA registered disinfectant effective against Norovirus available on-site at the food service establishment when responding to vomiting or diarrheal events that involve the discharge of vomitus or fecal matter onto surfaces in the food service establishment. Advised PIC to obtain an approved disinfectant from EPA List G.
Food stored covered — Inspector Note: Observed multiple foods uncovered in RIF (onion rings, cooked shrimp, and burger patty), in WIC (multiple containers of chicken eggroll, 1 container of celery sticks, 1 container of broccoli), and prep table (1 container of carrott sticks and 1 container of celery sticks). Advised PIC that except when cooling, food items shall be covered in order to protect from contamination. Advised PIC to place covers on all items.
