Observed several prepackaged TCS food items (2-containers of sour cream -expiration: 7/6/26 and 3/2/26, ranch dressing -expiration: 5/2026, and whipped cream - expiration: 2/6/2026) held beyond manufacturer's use by dates in refrigerator 2 in main kitchen. 2. Observed prepackaged shredded cheddar, mozzarella cheese, and pepperoni stored in refrigerator 1 in main kitchen without date mark. Per manufacturer's instructions on package, cheese must used within 7-days. C/A: Prepackaged sandwiches, eggs, infant formula, shucked oysters, milk, and time/temperature control safety foods that are labeled as “keep refrigerated” and that are for sale or service to the consumer or used as an ingredient in other foods shall be immediately discarded and shall not be sold, served, or used after the manufacturer’s expiration date, use by date, or the sell-by date. COS: All discarded.
Physical facilities installed, maintained, and clean — Inspector Note: Observed several pieces of unnecessary equipment (popcorn machine, etc.), appliances not in use, and empty boxes stored in main kitchen. C/A: The premises shall be free of items that are unnecessary to the operation or maintenance of the establishment, such as litter or equipment that is nonfunctional or no longer used and litter.
Food-contact surfaces: cleaned & sanitized — Inspector Note: 1. Observed thick, pink accumulations inside only bar gun nozzle at bar. 2. Observed dishes stored as clean in soiled cabinets in drawers of prep table in main kitchen. C/A: Equipment food-contact surfaces and utensils shall be clean to sight and touch. COS: Bar gun cleaned and sanitized; dishes moved to 3-compartment sink to be cleaned and sanitized.
Adequate ventilation and lighting; designated areas used — Inspector Note: Observed employee food stored on top of food for service in refrigerator in main kitchen. C/A: Areas designated for employees to eat, drink, store food, use tobacco products and electronic devices shall be located so that food, equipment, linens, and single-service and single-use articles are protected from contamination. COS: Food stored properly.
Certified Food Protection Manager — Inspector Note: Observed no CFSM certificate posted. PIC states previous food manager is no longer working at facility. C/A: Obtain CFSM within 60-days; Food service establishments shall have in its employ a Certified Food Safety Manager (CFSM) as specified in paragraph (b) of this subsection to ensure food safety is being managed within the food service establishment during all hours of operation as specified within paragraph (d) of this subsection. Email CFSM certificate to EHS within 60-days. COS-3/5/26 Informal Complaint investigation: EHS observed current CFSM certificated posted along front food service area for Vanessa Berryhill.
Proper date marking and disposition — Inspector Note: Observed sliced tomato, sliced iceberg lettuce, and cooked mushrooms stored in refrigerator in main kitchen more than 24-hours without a date mark. C/A: Refrigerated, ready-to-eat, time/temperature control for safety food prepared and held in a food establishment for more than 24 hours shall be clearly marked to indicate the date or day by which the food shall be consumed on the premises, sold, or discarded, when held at a temperature of 41°F (5°C) or below for a maximum of 7 days. COS: Discarded.
