Physical facilities installed, maintained, and clean — Inspector Note: Observed the WIF in the main kitchen with severe ice build-up on the edge of the door. Ice layer was so thick it prevented the door to close properly the door had to be slammed really hard for the latch to close C/A Good Repair. All physical facilities shall be maintained in good repair.

PIC present, demonstrates knowledge, performs duties — Inspector Note: Observed facility does have a key drop policy however there are no temperatures written on the invoice and manager is not monitoring that. Observed no signed allergen awareness training in the facility. Facility needs to update the employee health policy as well C/A PIC needs to monitor the key drop policy as well as training in allergen awareness C/A: An allergen awareness form was left with the shift manager
