Food and nonfood-contact surfaces cleanable, properly designed, constructed, and used — Inspector Note: Observed broken, missing floor tiles with accumulated ice inside WIF in main kitchen. C/A: Nonfood-contact surfaces of equipment that are exposed to splash, spillage, or other food soiling or that require frequent cleaning shall be constructed of a corrosion-resistant, nonabsorbent, and smooth material.

Insects, rodents, and animals not present — Inspector Note: 1. Observed several live and dead German cockroaches along floor and wall near prep top coolers primarily near pizza area in main kitchen. 2. Observed many broken, missing floor tiles throughout main kitchen pooling water and food debris providing harborage conditions for pests. C/A: The presence of insects, rodents, and other pests shall be controlled to minimize their presence on the premises by: 1. Routinely inspecting incoming shipments of food and supplies; 2. Routinely inspecting the premises for evidence of pests; 3. Using methods, if pests are found, such as trapping devices or other means of pest control as specified under subsections (6)(e), (6)(m), and (6)(n) of this Rule; Pf and 4. Eliminating harborage conditions. *Facility has active pest control service with Ecolab and must ensure pest control methods are effective and increase if necessary. *Due to ongoing issue with broken tiles, EHS continues to recommends floor repairs be completed within 30-days.

Insects, rodents, and animals not present — Inspector Note: Observed fly ribbons with dead flies stored above mixers and prep tables in dry storage/prep area in main kitchen. C/A: Insect control devices shall be installed so that: (i) The devices are not located over a food preparation area; and (ii) Dead insects and insect fragments are prevented from contact with exposed food; clean equipment, utensils, and linens; and unwrapped single-service and single-use articles. COS: Fly ribbons removed.

Food stored covered — Inspector Note: Observed several food items stored uncovered in prep top 1 (2-containers of flour), in WIF (breaded food items and raw shrimp on trays), and in RIF (2-containers of cheese sticks), in WIC #1 (meat sauce and cooked chicken wings), in prep top cooler 2(raw salmon, raw ribeye, risotto) all in main kitchen. C/A: Unless cooling, all food must be stored covered. COS: All food covered.

Food-contact surfaces: cleaned & sanitized — Inspector Note: 1. Observed many dishes stored as clean with accumulated food debris and splatter on shelving in dry storage area in main kitchen. 2. Observed thick, black accumulation along interior deflector inside ice machine in main kitchen. 3. Observed knives with residue stored as clean on wall knife rack with rust and dirt in main kitchen prep areas. 4. Observed in-use can opener blade heavily soiled with thick food residue at prep table in prep area of main kitchen. C/A: Equipment food-contact surfaces and utensils shall be clean to sight and touch. COS: All equipment cleaned and sanitized during the inspection.
