1 recorded inspectionFood and nonfood-contact surfaces cleanable, properly designed, constructed, and used — Inspector Note: Observed the gasket to the bottom of the prep top unit in the paradise grill area that was almost detaching from the door. Observed the walk in freezer door in the mexican side kitchen not closing properly and ice was buildup preventing the door to close properly Observed the self closing bracket to rear kitchen door is broken. Observed standing water on the floor in both sides of the kitchen. C/A Equipment shall be maintained in a state of repair and condition that meets the requirements specified under subsections (1) and (2)of this Rule. Gasket needs to be repaired/replaced in order to prevent cold air from leaking out and warm air getting in Equipment components such as doors, seals, hinges, fasteners, and kick plates shall be kept intact, tight, and adjusted in accordance with manufacturer's specifications. COS Facility has 72 hours to correct the violation
Food and nonfood-contact surfaces cleanable, properly designed, constructed, and used — Inspector Note: Observed the RIC in the South of the border area with an ambient temperature of 52*F. Observed three RIC in the Paradise Grill Area with an ambient temperature of 51*F Observed the prep top unit in the Paradise grill area with an ambient temperature of 49*F. Observed the Cooling Drawers below the grill in the On The border kitchen with an ambient temperature of 44*F C/A Good Repair and Proper Adjustment. 1.Ambient air temperature, water pressure, and water temperature measuring devices shall be maintained in good repair and be accurate within the intended range of use. COS: Facility has 72 hours to correct the violation. Cooling drawers were unplugged at night and they were plugged in UPDATE: Facility has sent the invoice that the units have been repaired. Food and nonfood-contact surfaces cleanable, properly designed, constructed, and used — Inspector Note: Observed the storage racks in the south of the border and the paradise grill area with a build-up of heavy debris. Observed the fan cover in the WIC and WIF in paradise grill area with black debris build-up. Observed the fan cover in the WIC with black debris build-up in the south of the border area. C/A Nonfood-contact Surfaces. Nonfood-contact surfaces shall be free of unnecessary ledges, projections, and crevices, and designed and constructed to allow easy cleaning and to facilitate maintenance. COS: Facility has 72 hours to correct the violation
Physical facilities installed, maintained, and clean — Inspector Note: Observed the quarry tiles in the Paradise grill area are loose and lifting off and the grout is missing in multiple places. Water was seeping underneath of the tile creating a perfect harborage for gnats and other insects. C/A Good Repair. All physical facilities shall be maintained in good repair. 1. The physical facilities shall be cleaned as often as necessary to keep them clean and by methods that prevent contamination of food products. COS: Facility has 72 hours to correct the violation
Food stored covered — Inspector Note: Observed numerous food in the WIF (salsa Verde, Grilled Diced Chicken, Cooked shredded beef, melted queso, cooked ground beef) a total of 16 containers that were placed in to cool on 2/25 and they were not covered after the food cooled. C/A Except as specified under paragraph 2. of this subsection and when cooling as specified in DPH Rule 511-6-1-.04(6)(e)2.(ii), storing the food in packages, covered containers, or wrappings, except for loosely covered or uncovered containers in which food is being cooled if protected from overhead contamination; COS: Food was covered
Food-contact surfaces: cleaned & sanitized — Inspector Note: Observed the ice bin in the American side next to the smoking bar area with a build-up of some type of pink/black build-up. The ice was touching the the build/up C/A In equipment such as ice bins and beverage dispensing nozzles and enclosed components of equipment such as ice makers, cooking oil storage tanks and distribution lines, beverage and syrup dispensing lines or tubes, coffee bean grinders, and water vending equipment: (I) At a frequency specified by the manufacturer; COS: Facility removed the ice and sanitize the bin
Proper cold holding temperatures — Inspector Note: Observed several TCS foods on prep top coolers in both kitchens and the walk in cooler in both sides (South of the border/paradise grill) the RIC in the Paradise Grill side were cold holding above 41F. These items were marked with an asterisk below. CA: Except during preparation, cooking, or cooling, or when time is used as the public health control, time/temperature control for safety food shall be maintained at 41°F (5°C) or below when cold holding. COS: Food items were discarded except for items that had recently been placed on the prep top about 1 hour ago on the border side kitchen. These items were placed in the freezer for rapid cooling. Proper cooling time and temperature — Inspector Note: Observed the grilled chicken and the sliced tomatoes that were stored in the RIC in the paradise grill area the cooling rate was not fast enough for the food to reach below 41*f in 4 hours C/A Time/temperature control for safety food shall be cooled within 4 hours to 41°F (5°C) or less COS: Chicken was discarded and the tomatoes were placed in the WIC
