Physical facilities installed, maintained, and clean — Inspector Note: Observed soiled floor throughout dry storage area in main kitchen with accumulated dirt and debris behind RIFs(2). C/A: The physical facilities shall be cleaned as often as necessary to keep them clean and by methods that prevent contamination of food products.

Item 18 Good Retail Practice Corrected On Site (COS)

Insects, rodents, and animals not present — Inspector Note: Observed large opening along bottom and side of exit door in main kitchen allowing potential entry of pests. C/A: Outer openings of a food service establishment shall be protected against the entry of insects and rodents by: (i) Filling or closing holes and other gaps along floors, walls, and ceilings; (ii) Closed, tight-fitting windows; and (iii) Solid, self-closing, tight-fitting doors. *PIC and EHS discussed replacing exit door within 2-3weeks; PIC will send photo when door is repaired. COS-4/8/26: Operator provided photo confirmation of replaced exit door; violation corrected.

Insects, rodents, and animals not present — Inspector Note: Observed live roaches along upper wall above 3-compartment sink in main kitchen. C/A: The presence of insects, rodents, and other pests shall be controlled to minimize their presence on the premises by: 1. Routinely inspecting incoming shipments of food and supplies; 2. Routinely inspecting the premises for evidence of pests; 3. Using methods, if pests are found, such as trapping devices or other means of pest control as specified under subsections (6)(e), (6)(m), and (6)(n) of this Rule; and 4. Eliminating harborage conditions. *Employee cleaned wall area; facility must eliminate ALL harborage conditions and increase pest control measures.

Proper cold holding temperatures — Inspector Note: Observed several TCS foods cold holding at above 41F in main kitchen (*see asterisks*). C/A: All TCS foods cold held must be maintained at 41F and below. COS: Per PIC, chicken wings were removed from WIC less than 30-minutes ago; wings placed on TPHC. Blue cheese single cups were refilled less than 1-hour ago per PIC; all placed on ice - see temperature section for corrective action re-tempts.
