In-use utensils: properly stored — Inspector Note: [1] Observed scoops in the container or poppy seeds and shaved onions and other dry seasonings with the handles touching the food. [2] Observed in-use utensils (grill scrapes and knives) stored in stagnant water with a temperature at 81F. Insects, rodents, and animals not present — Inspector Note: Observed 3 live German roaches and 1 dead German roach near the bulk dry storage bins located directly underneath the oven in the main kitchen area. C/A: The presence of insects, rodents, and other pests shall be controlled to minimize their presence on the premises by: 1. Routinely inspecting incoming shipments of food and supplies; 2. Routinely inspecting the premises for evidence of pests; 3. Using methods, if pests are found, such as trapping devices or other means of pest control as specified under subsections (6)(e), (6)(m), and (6)(n) of this Rule; Pf and 4. Eliminating harborage conditions PIC must request additional pest control methods/applications to eliminate presence of German cockroaches in facility and send the report to EHS by July 23rd, 2026.
