Observed floor under cornstarch rack, inside employee restroom, and in WIC heavily soiled with thick black residue and dirt in main kitchen. 2. Observed missing baseboards and holes in the wall of employee restroom in main kitchen. C/A: Repair baseboards and holes in wall and clean all floors. All physical facilities shall be maintained in good repair. The physical facilities shall be cleaned as often as necessary to keep them clean and by methods that prevent contamination of food products.
