Proper cold holding temperatures — Inspector Note: Observed several items cold holding above 41F in pizza area prep top cooler, prep top cooler 1 in main kitchen, and prep top cooler 4 next to grill in main kitchen (*see asterisks*). CA: when cold holding, all TCS food items shall be maintained at 41F or below. COS: All items were discarded.

Food in good condition, safe, and unadulterated — Inspector Note: Observed a container of molded strawberries in a prep top unit located in the main kitchen. Observed sliced tomatoes with mold on the in a prep top unit located in the main kitchen. Observed 2 boxes of tomatoes extremely soft to the touch with mold-like growth on them. C/A: Food shall be safe, unadulterated, and honestly presented COS: items discarded

Food-contact surfaces: cleaned & sanitized — Inspector Note: Observed right bulk ice machine located in the main kitchen with pink build-up on the interior metal chute and black and pink build-up on the interior white chute. C/A: In equipment such as ice bins and beverage dispensing nozzles and enclosed components of equipment such as ice makers, cooking oil storage tanks and distribution lines, beverage and syrup dispensing lines or tubes, coffee bean grinders, and water vending equipment: (I) At a frequency specified by the manufacturer; or (II) Absent manufacturer specifications, at a frequency necessary to preclude accumulation of soil or mold.
