Physical facilities installed, maintained, and clean — Inspector Note: Observed a prep top cooler and a 2-door reach-in cooler located in the main kitchen not in use and labeled as "out of order". Observed a 2-door reach-in cooler located in the vegetable prep area not in use and labeled as "out of order". Per CFSM "units have been out of order for the last 2 years". C/A: The premises shall be free of items that are unnecessary to the operation or maintenance of the establishment, such as litter or equipment that is nonfunctional or no longer used and litter.

Food stored covered — Inspector Note: Observed a container or cook pork stored uncovered in the reach-in cooler located in the main kitchen area. Observed baked beans, cooked salmon, and roasted brussels sprouts stored uncovered in walk-in cooler (WIC) #1 located in the main kitchen area. C/A: food shall be stored covered; except for loosely covered or uncovered containers in which food is being cooled if protected from overhead contamination COS: items covered with a lid
