Posted: Permit/Inspection/Choking Poster/Handwashing — Inspector Note: Observed no inspection report posted in the drive-thru window for customers to view. CA: Food service establishments with drive-thru windows will post the current inspection report, and also have the inspection report posted so that a minimum of the top one-third of a copy of the current inspection report is visible through each window allowing customers to easily read the score, date of inspection and establishment information.

Physical facilities installed, maintained, and clean — Inspector Note: Observed ceiling tile missing and exposing the ventilation above the ice machine in the main kitchen. CA: All physical facilities shall be maintained in good repair

Insects, rodents, and animals not present — Inspector Note: Observed an overwhelming presence of fruit flies throughout the facility. Observed fruit flies inside of mango containers in the prep top cooler in the front food service area. Observed fruit flies on prep counters in the main kitchen. Observed gnats and fruit flies on the tables in the dining area in the front service area. CA: The presence of insects, rodents, and other pests shall be controlled to minimize their presence on the premises by: 1. Routinely inspecting incoming shipments of food and supplies; 2. Routinely inspecting the premises for evidence of pests; 3. Using methods, if pests are found, such as trapping devices or other means of pest control as specified under subsections (6)(e), (6)(m), and (6)(n) of this Rule; Pf and 4. Eliminating harborage conditions.

PIC present, demonstrates knowledge, performs duties — Inspector Note: Food employee informed EHS that there was no PIC present. Food employee had to call PIC in order to find documents, etc. PIC stated that they are usually always there, so there is no other PIC designated. CA: There must be a person in charge on the premises of the food service establishment at all times to ensure food safety.

PIC present, demonstrates knowledge, performs duties — Inspector Note: PIC informed EHS, via phone call, that they receive deliveries while there is no employees present at the facility. PIC stated that they have access to codes to get into the facility while the facility is closed. PIC could not provide a document or signed agreement to show how key-drop deliveries are monitored. CA: Written procedures and plans, where specified by this Chapter and as developed by the food service establishment, are maintained and implemented as required.

Food stored covered — Inspector Note: Observed ice for smoothies stored uncovered in the prep counter in the front service area. Observed containers of berries, banana, mango and boba stored uncovered in the prep top cooler in the front service area. CA: Except as specified under paragraph 2. of this subsection and when cooling as specified in DPH Rule 511-6-1-.04(6)(e)2.(ii), storing the food in packages, covered containers, or wrappings, except for loosely covered or uncovered containers in which food is being cooled if protected from overhead contamination. COS: Food items were covered with the appropriate lid.
