Physical facilities installed, maintained, and clean — Inspector Note: Broken floor tiles were observed throughout the kitchen. Holes were observed in the walls inside the back kitchen near the hand sink. CA Physical facilities must be maintained and in good repair.

COS TCS food items were discarded and transferred into the prep units stored on the front prep line and kitchen where temperatures were maintained at 41F.
