A container of seasoning and rice stored inside the kitchen was observed not labeled. CA: Food packaged in a food service establishment, that is not provided by a foodservice employee upon consumer request, shall be labeled. The label information shall include the common name of the food, or if there is no common name, an adequately descriptive identity statement.

Physical facilities installed, maintained, and clean — Inspector Note: Mold was observed on the wall above the 3-compartment sink. CA: All physical facilities shall be maintained in good repair.

Food stored covered — Inspector Note: Observed shrimp, chicken tenders, and lamb stored inside the reach in freezer uncovered. A container of celery stored inside the walk in cooler was stored uncovered. CA: Storing the food in packages, covered containers, or wrappings, except for loosely covered or uncovered containers in which food is being cooled if protected from overhead contamination. COS: The food items were covered.

Food-contact surfaces: cleaned & sanitized — Inspector Note: Mold was observed inside the ice machine on the shield and side of the unit in the kitchen. Mold was observed inside the soda dispenser where the nozzles are placed. Food debris was observed inside the blade of the vegetable slicers stored underneath the prep unit inside the kitchen. CA: Equipment food-contact surfaces and utensils shall be clean to sight and touch. COS: Equipment was cleaned and sanitized.

Food-contact surfaces: cleaned & sanitized — Inspector Note: The concentration of the sanitizer bucket stored inside the kitchen tested at 0PPM for chlorine. CA: A chemical sanitizer used in a sanitizing solution for a manual or mechanical operation at contact times shall be used in accordance with the Environmental Protection Agency (EPA)-registered label use instructions. COS: The concentration of the chlorine tested at 50PPM.

Proper cold holding temperatures — Inspector Note: Observed lettuce, tomatoes, ham stored inside the prep unit closest to the front counter held cold holding temperatures above 41F. Cheese and rice stored inside the prep unit closest to the grill held cold holding temperatures above 41F. CA: Except during preparation, cooking, or cooling, or when time is used as the public health control, time/temperature control for safety food shall be maintained at 41°F (5°C). COS: Food items stored for 30 minutes was placed in ice. The rice and provolone cheese inside the prep unit closest to the grill was discarded.
