C/A The most current inspection report shall be prominently displayed in public view at all times, within fifteen feet of the front or primary public door and between five feet and seven feet from the floor and in an area where it can be read at a distance of one foot away.
Posted: Permit/Inspection/Choking Poster/Handwashing — Inspector Note: Observed facility does not have a handwashing sign nor a handwashing poster by the hand sink in the kitchen area C/A Handwashing Signage. A sign or poster that notifies food employees to wash their hands shall be provided at all handwashing sinks used by food employees and shall be clearly visible to food employees. COS Facility has 72 hours to correct the violation
Warewashing facilities: installed, maintained, used; test strips — Inspector Note: Observed dishmachine at the bar area was not dispensing sanitizer. Tested at 0 ppm. C/A: Have dishmachine serviced to dispense Chlorine sanitizer at 50 ppm - 100 ppm. COS All equipment must be washed at 3-compartment sink until the unit is repaired. Facility has 72 hours to correct the violation
Toilet facilities: properly constructed, supplied, cleaned — Inspector Note: Observed two out of three toilet stalls without any toilet paper. C/A Toilet Tissue, Availability. A supply of toilet tissue shall be available at each toilet. Pf COS Facility has 72 hours to correct the violation
Management knowledge, responsibilities, reporting — Inspector Note: Observed employees do not have a verifiable manner on the premises to demonstrate knowledge of their responsibility to report symptoms and illnesses related to FBI. CA: Food employees and conditional employees are informed in a verifiable manner of their responsibility to report in accordance with the chapter, to the person in charge, information about their health and activities as they relate to diseases that are transmissible through food. Adequate handwashing facilities supplied & accessible — Inspector Note: Observed the handsink in the kitchen area without paper towels or any other device to dry their hands. C/A Hand Drying Provision. Each handwashing sink or group of adjacent handwashing sinks shall be provided with: 1. Individual, disposable towels; Pf 2. Response procedures for vomiting & diarrheal events — Inspector Note: Observed facility does not have a fecal and vomit procedure anywhere in the facility. C/A Responding To Contamination Events. A food establishment shall have written procedures for employees to follow and an EPA registered disinfectant effective against Norovirus available on-site at the food service establishment when responding to vomiting or diarrheal events that involve the discharge of vomitus or fecal matter onto surfaces in the food service establishment. The procedures shall address the specific actions employees must take to minimize the spread of contamination and the exposure of employees, consumers, food, and surfaces to vomitus or fecal matter.Pf COS: EHS handed the owner a procedure to laminate and posted in the kitchen area
Food-contact surfaces: cleaned & sanitized — Inspector Note: Observed the ice bin with a accumulation of pink and black substance. C/A In equipment such as ice bins and beverage dispensing nozzles and enclosed components of equipment such as ice makers, cooking oil storage tanks and distribution lines, beverage and syrup dispensing lines or tubes, coffee bean grinders, and water vending equipment: (I) At a frequency specified by the manufacturer; or (II) Absent manufacturer specifications, at a frequency necessary to preclude accumulation of soil or mold. COS: Facility has 72 hours to correct the violation
