1 recorded inspectionFood properly labeled; original container — Inspector Note: Observed several foods stored at the bottom of the prep top in the main kitchen without a proper label on them ( ranch dressing, raspberry vinegar, Italian dressing). Observed a large clear container on the shelf in the main kitchen without a label identifying the produce inside of it. C/A Anytime a food is removed from its original package the food shall be identified by (i) The common name of the food, or if there is no common name, an adequately descriptive identity statement; COS Facility has 3 days to correct the violation
Contamination prevented during food preparation, storage, display — Inspector Note: Observed a large container of ice tea stored directly on the floor in the WIC in the main kitchen. C/A Food Storage. 1. Except as specified in paragraphs 2 and 3 of this subsection, food shall be protected from contamination by storing the food: (i) In a clean, dry location; (ii) Where it is not exposed to splash, dust, or other contamination; and (iii) At least 6 inches (15 cm) above the floor. COS: Manager removed the tea off the floor
PIC present, demonstrates knowledge, performs duties — Inspector Note: Observed one employee that was hired 8 months ago not being aware which are the 5 symptoms and 6 illnesses that he is required to report to his manager. Observed facility has no verifiable manner that employees are being trained on cross contamination and which one are the 9 allergies they should be aware of despite having a posted on the board in the main kitchen. C/A Food Safety Training. Employees are properly trained in food safety, including food allergy awareness, as it relates to their assigned duties. Food allergy awareness includes describing foods identified as major food allergens and the symptoms that a major food allergen could cause in a sensitive individual who has an allergic reaction;Pf (o) Reporting Responsibilities. Food employees and conditional employees are informed in a verifiable manner of their responsibility to report in accordance with the Chapter, to the person in charge, information about their health and activities as they relate to diseases that are transmissible through food;Pf COS: Allergy awareness form as well as employee health policy was given to the manager and have the employees sign it
Food stored covered — Inspector Note: Observed two trays of Jello and sliced cake stored on the rolling shelf in the WIC in the main kitchen uncovered. C/A Except as specified under paragraph 2. of this subsection and when cooling as specified in DPH Rule 511-6-1-.04(6)(e)2.(ii), storing the food in packages, covered containers, or wrappings, except for loosely covered or uncovered containers in which food is being cooled if protected from overhead contamination; COS Food was covered to prevent from overhead contamination
Food-contact surfaces: cleaned & sanitized — Inspector Note: Observed ice bin having some type of black slime accumulation in the crevice's near the chute. Observed a box of ground beef, sherbet, diced chicken meat stored directly on the floor in the mobile freezer unit. C/A In equipment such as ice bins and beverage dispensing nozzles and enclosed components of equipment such as ice makers, cooking oil storage tanks and distribution lines, beverage and syrup dispensing lines or tubes, coffee bean grinders, and water vending equipment: (I) At a frequency specified by the manufacturer; or (II) Absent manufacturer specifications, at a frequency necessary to preclude accumulation of soil or mold. COS: Maintenance was made aware and cleaned the ice bin on the spot
