Toilet facilities: properly constructed, supplied, cleaned — Inspector Note: Observed men and women restrooms in lobby without self closing doors. C/A: A toilet room located on the premises shall be completely enclosed and provided with a tight-fitting and self-closing door. 6/4/2026: Operators provided video evidence of corrections to EHS via email; doors observed self closing.

Physical facilities installed, maintained, and clean — Inspector Note: Observed porous, absorbent ceiling tiles in men and women restroom in lobby. C/A: Materials for indoor floor, wall, and ceiling surfaces under conditions of normal use shall be: 1. Smooth, durable, and easily cleanable for areas where food service establishment operations are conducted; 2. Closely woven and easily cleanable carpet for carpeted areas; and 3. Nonabsorbent for areas subject to moisture such as food preparation areas, walk-in refrigerators, warewashing areas, toilet rooms, mobile food service unit servicing areas, and areas subject to flushing or spray cleaning methods. 6/4/2026: Operators provided photo evidence of corrections to EHS via email; ceiling tiles observed in compliance in both areas.
