Nonfood-contact surfaces clean — Inspector Note: Observed build up of grease and debris on steam hood and outer areas of cooking equipment (fryers, flat top). CA. PIC advised to clean equipment. Nonfood-contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residues.

Physical facilities installed, maintained, and clean — Inspector Note: Observed accumulated dust on vents and ceiling tiles around vents in main kitchen. Also observed missing ceiling tiles in main kitchen. CA. The physical facilities shall be cleaned as often as necessary to keep them clean and by methods that prevent contamination of food products. All physical facilities shall also be maintained in good repair. PIC advised to clean vents and replace missing ceiling tiles.

Insects, rodents, and animals not present — Inspector Note: Observed hole in wall in main kitchen and open ceiling in small storage in hallway to walk in cooler. Also observed glue trap on floor in main kitchen. PIC reported pest control is conducted once per month. No pest control reports observed. CA. PIC advised to seal all openings which may be entry points for pests, and provide most recent pest control report.
