Nonfood-contact surfaces clean — Inspector Note: OBSERVATION: Interior of the prep top cooler top lid, interior & exterior of the Delfield freezer, walk-in cooler metal rolling racks are unclean to sight and touch with food accumulation. Floor mat in the walk-in freezer is unclean with debris. CA: 3. Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris.

Except as specified in paragraphs 2 through 5 of this subsection, outer openings of a food service establishment shall be protected against the entry of insects and rodents by: (i) Filling or closing holes and other gaps along floors, walls, and ceilings; (ii) Closed, tight-fitting windows; and (iii) Solid, self-closing, tight-fitting doors.

Management knowledge, responsibilities, reporting — Inspector Note: Signed employee health policy are used for training, and were not observed for 3 onsite staff. CA: Food employees and conditional employees are informed in a verifiable manner of their responsibility to report in accordance with the Chapter, to the person in charge, information about their health and activities as they relate to diseases that are transmissible through food. Discussed with PIC to train employees on their responsibilities of the employee to report any of the 5 symptoms/6 diseases and to have a verifiable method of training conducted.
