CA: Advised PIC Except as specified in paragraphs 2 through 5 of this subsection, outer openings of a food service establishment shall be protected against the entry of insects and rodents by: (i) Filling or closing holes and other gaps along floors, walls, and ceilings.

Adequate handwashing facilities supplied & accessible — Inspector Note: Observed a small purple cutting board stored inside the hand sink located at the server station in the main kitchen. CA: Advised PIC a handwashing facility may not be used for purposes other than handwashing. COS: PIC removed the cutting board and placed it inside the dish pit

Food separated and protected — Inspector Note: Observed raw chicken stored behind a container of cook chicken on the prep top cooler and a container of cooked beef stored on top of a container of raw chicken inside the 2-door standing reach-in cooler on the cook line. CA: Advised PIC food shall be protected from cross contamination by: arranging each type of food in equipment so that cross contamination of one type with another is prevented. COS: PIC re-arranged food items during time of inspection
