CA: The physical facilities shall be cleaned as often as necessary to keep them clean and by methods that prevent contamination of food products. COS: PIC had the spill cleaned from the floor.

CA: Dead or trapped birds, insects, rodents, and other pests shall be removed from control devices and the premises at a frequency that prevents their accumulation, decomposition, or the attraction of pests. PIC removed all dead pests.

Nonfood-contact surfaces clean — Inspector Note: Observed multiple shelves in the walk-in-cooler with a build-up of organic matter. CA: Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris. COS: PIC had an employee clean shelves.

Food-contact surfaces: cleaned & sanitized — Inspector Note: Observed the two cups of sanitizer solution (used to sanitize the probes of thermometers after temping food) at 0ppm quat residual. CA: Quat sanitizer shall be made to the manufacturer's recommended concentration for food contact surfaces. COS: PIC remade the sanitizer solutions to 200ppm quat. Advised to re-train employees on how to make and check the sanitizer solution concentration, for food contact surfaces.

Item 15A Good Retail Practice

Food and nonfood-contact surfaces cleanable, properly designed, constructed, and used — Inspector Note: Observed the ambient temperature of the 1 Door Pass Thru Cooler (closest to the mop sink) at 45F. The foods in the cooler had just been placed in there this morning. CA: 1. Equipment shall be maintained in a state of repair and condition that meets the requirements specified under subsections (1) and (2)of this Rule. PIC moved the foods to a working cooler. Advised PIC not to use this cooler to hold any TCS foods until it is serviced and holding at 41F or lower (ideally 36F to 38F). Facility still has adequate refrigeration available.

Insects, rodents, and animals not present — Inspector Note: Observed a heavy presence of live, small black flies (the kind that sit on the walls and ceiling) throughout the facility. CA: Facility shall effectively control pests within the establishment. Advised to work with pest control to target that pest specifically. Advised PIC to email the most recent pest control service report to the health authority by 11/10/25.
