Item 6-1A Critical / Risk Factor Corrected On Site (COS)

Proper cold holding temperatures — Inspector Note: Observed multiple TCS foods items cold holding at temperatures above 41F, located in the Large Prep Top Cooler (see temperature log - foods marked with asterisks). Observed the ambient temperature of the Large Prep Top Cooler at 48F. PIC stated the TCS foods had been in the top of cooler had been stocked this morning. CA: Except during preparation, cooking, or cooling, or when time is used as the public health control, time/temperature control for safety food shall be maintained at 41°F (5°C) or below or 135°F (57°C) or above. COS: PIC had employees put all TCS foods in top part of cooler on an ice bath. TCS foods stored in cooler overnight were discarded. Advised PIC not to use the cooler to hold TCS foods until the cooler has an ambient temperature below 41F (ideal ambient temperature is between 36F-38F).

Item 15A Good Retail Practice Corrected On Site (COS)

Food and nonfood-contact surfaces cleanable, properly designed, constructed, and used — Inspector Note: Observed several untensils in disrepair. Observed 2 plastic scoops, stored inside bulk food bins, with the handles broken off. Observed a spatula, handing up near 2-compartment sink, with a piece broken off the food-contact part. CA: Utensils shall be maintained in a state of repair or condition that complies with the requirements specified under subsections (1) and (2) of this Rule or shall be discarded. COS: Both scoops and the spatula were discarded by the PIC.

Nonfood-contact surfaces clean — Inspector Note: Observed multiple lower shelves of prep tables unclean to sight and touch. CA: Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris. Advised to thoroughly clean and sanitize these surfaces by 12/17/25.

Food-contact surfaces: cleaned & sanitized — Inspector Note: Observed the blade area of the deli slicer with dried food debris. CA: Equipment food-contact surfaces and utensils shall be clean to sight and touch. COS: PIC disassembled the deli slicer and thoroughly washed, rinsed and sanitized.
