Item 16B Good Retail Practice

Plumbing installed; proper backflow devices — Inspector Note: System Maintained in Good Repair. A plumbing system shall be repaired according to law; and maintained in good repair. Observed leak at indirect waste line on hand wash sink in Dessert / Soup prep area. PIC was advised to have leak repaired by a licensed plumber and email invoice to inspector by 12/15/25.

Item 2-2D Critical / Risk Factor Corrected On Site (COS)

Adequate handwashing facilities supplied & accessible — Inspector Note: Using a Handwashing Sink. A handwashing sink shall be maintained so that it is accessible at all times for employee use. A handwashing facility may not be used for purposes other than handwashing. Observed one used, wet wiping cloth and one knife stored directly in hand wash sink opposite to ovens in Front food prep and service area. COS: Said items were removed by food worker and sink was washed rinsed and sanitized.

Food in good condition, safe, and unadulterated — Inspector Note: Food shall be safe, unadulterated, and honestly presented. Observed two dead fruit flies in direct contact with alcohol in half bottle of Johnnie Walker Red stored on liquor storage shelf in bar area. COS: Food item was discarded by PIC.

Item 4-2B Critical / Risk Factor

Food-contact surfaces: cleaned & sanitized — Inspector Note: Except as specified in paragraph 2 of this subsection, in a mechanical operation, the temperature of the fresh hot water sanitizing rinse as it enters the manifold may not be more than 194ºF (90ºC), or less than: (i) For a stationary rack, single temperature machine, 165ºF (74ºC); or (ii) For all other machines, 180ºF (82ºC). Observed final rinse temperature tested at range between 152 - 154F after several attempts to reboot said equipment. PIC was advised to have said equipment serviced by ECOLAD ASAP and corrected no later than 12/15/2025 and send invoice to inspector via email. In the interim the 3 compartment sink will have to be used to wash rinse and sanitize dishes.

Food-contact surfaces: cleaned & sanitized — Inspector Note: Manual and Mechanical Equipment, Chemical Sanitization - Temperature, pH, Concentration, and Hardness. A chemical sanitizer used in a sanitizing solution for a manual or mechanical operation at contact times specified under subsection (8)(b)3 of this Rule shall meet the requirements specified in DPH Rule 511-6-1-.07(6)(g), shall be used in accordance with the Environmental Protection Agency (EPA)-registered label use instructions. Observed no chlorine sanitizer testing at 10 ppm at dish washer in the coffee prep area and no reading available for glass dishwasher in the bar area. PIC was advised to have ECOLAB service machines ASAP and send invoice via email to inspector by 12/15/25.
