Posted: Permit/Inspection/Choking Poster/Handwashing — Inspector Note: Observed no food service permit displayed. CA: Post the permit in a location in the food service establishment that is conspicuous to consumers. COS: PIC printed and displayed permit.

Item 4-2B Critical / Risk Factor Corrected On Site (COS)

Food-contact surfaces: cleaned & sanitized — Inspector Note: Observed sanitizer dispenser for the 3 compartment sink not dispensing Quat sanitizer at the correct concentration. Observed 100ppm quat in the sanitizer solution in the sink, and in subsequent buckets used to test the dispenser. Appropriate concentration is 200ppm according to the manufacturer instructions. CA: A chemical sanitizer used in a sanitizing solution for a manual or mechanical operation at contact times specified under subsection (8)(b)3 of this Rule shall meet the requirements specified in DPH Rule 511-6-1-.07(6)(g), shall be used in accordance with the Environmental Protection Agency (EPA)-registered label use instructions P, and shall be used as follows: - A quaternary ammonium compound solution shall have a minimum temperature of 75ºF (24ºC), P have a concentration as specified in DPH Rule 511-6-1-.07(6)(g) and as indicated by the manufacturer’s use directions included in the labeling, and be used only in water with 500 ppm hardness or less or in water having a hardness no greater than specified by the EPA-registered label use instructions. COS: PIC manually dispensed sanitizer to create sanitizer solution of 200ppm. PIC instructed employees that sanitizer solution must be manually mixed and tested before each use until dispenser can be serviced. CORRECT BY: Having dispenser serviced to dispense the correct concentration of sanitizer. Send evidence of correction to inspector no later than 8/20/2026.

Personal cleanliness — Inspector Note: Except for a plain ring such as a wedding band food employees may not wear jewelry including medical information jewelry on their arms and hands while preparing food. Observed PIC wearing one ring and wrist watch while preparing food for customers in the front food prep and service area. COS : PIC removed said jewelry items , washed hands and donned new gloves.

Plumbing installed; proper backflow devices — Inspector Note: System Maintained in Good Repair. A plumbing system shall be repaired according to law; P and maintained in good repair. Observed liquid waste form hand wash sink leaking directly onto floor in the front food prep area. PIC was advised to have leak repaired by a licensed plumber and submit invoice to inspector by 1/2/26.

Equipment is used for storage of packaged or unpackaged food, such as a reach-in refrigerator, and the equipment is cleaned at a frequency necessary to preclude accumulation of soil residues. Observed pool of standing water on floor of two door low boy reach in refrigerator containing Milk shake mix in the front food service and preparation area. AS per PIC refrigeration technician was called to service said equipment. PIC was advised to send invoice to inspector via email by 1/9/2026.
