Posted: Permit/Inspection/Choking Poster/Handwashing — Inspector Note: Upon arrival, observed the current inspection had been moved from it's usual location and was located on a shelf near the front door, about 10 feet from the floor. CA: The most current inspection report shall be prominently displayed in public view at all times, within fifteen feet of the front or primary public door and between five feet and seven feet from the floor and in an area where it can be read at a distance of one foot away. COS: PIC moved the inspection report to a location that was within fifteen feet of the front or primary public door and between five feet and seven feet from the floor and in an area where it can be read at a distance of one foot away.

Food and nonfood-contact surfaces cleanable, properly designed, constructed, and used — Inspector Note: Observed the 2 top drawers of the 4 drawer cooler unable to stay closed. Employees are using towels (wedged between the door) to keep them closed between uses. CA: Equipment components such as doors, seals, hinges, fasteners, and kick plates shall be kept intact, tight, and adjusted in accordance with manufacturer's specifications. Advised to repair by 8.24.26.

COS: PIC placed a roll of paper towels at the handwashing sink.

Item 11C Good Retail Practice Corrected On Site (COS)

Approved thawing methods used — Inspector Note: Observed reduced oxygen packaged fish with a label indicating that it is to be removed from packaging prior to thawing thawing under refrigeration in the main kitchen. CA: Reduced oxygen packaged fish that bears a label indicating that it is to be kept frozen until time of use shall be removed from the reduced oxygen environment: (i) Prior to its thawing under refrigeration that maintains the food temperature at 41°F (5°C) or less; or (ii) Prior to, or immediately upon completion of its thawing, using procedures to completely submerge in running water as specified in paragraph 2 of this subsection. COS: ROP fish was still below freezing (31F). PIC removed fish from packaging and placed it back in the cooler.

Hands clean and properly washed — Inspector Note: Observed an employee not washing hands between handling raw meat and RTE foods. Employee changed gloves between tasks but did not wash hands between glove changes. CA: Food employees shall clean their hands and exposed portions of their arms immediately before engaging in food preparation including working with exposed food, clean equipment and utensils, and unwrapped single-service and single-use articles and: (i) After touching bare human body parts other than clean hands and clean, exposed arms; (ii) After using the toilet room; (iii) After caring for or handling service animals or aquatic animals; (iv) After coughing, sneezing, using a handkerchief or disposable tissue, using tobacco products or electronic devices that simulate tobacco smoking, eating or drinking, except for drinking from a closed beverage container and the container is handled to prevent contamination of the hands; (v) After handling soiled equipment or utensils; (vi) During food preparation, as often as necessary to remove soil and contamination and to prevent cross contamination when changing tasks; (vii) When switching between working with raw food and working with ready-to-eat food; (viii) Before donning gloves to initiate a task that involves working with food; and (ix) After engaging in other activities that contaminate the hands. COS: Employee washed his hands before putting on new gloves and changing tasks.

Food-contact surfaces: cleaned & sanitized — Inspector Note: Observed the service station ice machine, vegetable peelers stored clean, the inside of one soda gun, magnetic knife block and the inside of an iced tea urn stored clean unclean to sight and touch. CA: Equipment food-contact surfaces and utensils shall be clean to sight and touch. Correct by: Cleaning and sanitizing the above food contact surfaces. Photos of correction can be sent to inspector at [REDACTED EMAIL] by 8/3/26.

Proper cold holding temperatures — Inspector Note: Observed several TCS foods cold holding at temperatures above 41F. See ** in temperature log. CA: Time/Temperature Control for Safety Food, Hot and Cold Holding. Except during preparation, cooking, or cooling, or when time is used as the public health control, time/temperature control for safety food shall be maintained at 41°F (5°C) or below or 135°F (57°C) or above, except that roasts cooked to a temperature and for a time specified in subsection (5)(a)2 of this Rule and reheated using the same temperature and time conditions as cooking may be held at a temperature of 130°F (54°C) or above. COS: PIC discarded TCS foods out of temperature past 4 hours. Foods that could be cooled were placed in the walk-in cooler to cool back down to 41F or below.

Contamination prevented during food preparation, storage, display — Inspector Note: Observed condensers and piping dripping in the walk-in cooler. Recent food delivery was stored on the floor directly under dripping fan when inspectors arrived. CA: Food shall be protected from contamination by storing the food: (i) In a clean, dry location; (ii) Where it is not exposed to splash, dust, or other contamination; and (iii) At least 6 inches (15 cm) above the floor. COS: PIC moved recent food delivery off floor and into correct shelving to prevent contamination

Observed right-most prep top cooler with excessive condensation. CA: Equipment components such as doors, seals, hinges, fasteners, and kick plates shall be kept intact, tight, and adjusted in accordance with manufacturer's specifications. Correct by 11/14/25 - send proof of cooler repair to inspector
