2 recorded inspectionsNonfood-contact surfaces clean — Inspector Note: Observed wall soiled with accumulated food debris. Observed build-up of grease on nonfood-contact surface of the burners. Observed outside of metal containers holding cookie crumbles and grits unclean to sight. CA: Nonfood-contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residues.
Hot and cold water available; adequate pressure — Inspector Note: Observed the water pressure for the handwashing sink near the warewashing area, in the men's restroom, and in the women's restroom low. Observed no water water in handwashing sink next to 3-compartment sink when the 3-compartment sink is using water.
Physical facilities installed, maintained, and clean — Inspector Note: Observed excess food and grease build-up on the floors between equipment, appliances and on the floor of main kitchen area. CA: Except for cleaning that is necessary due to a spill or other accident, cleaning shall be done during periods when the least amount of food is exposed, such as after closing. COS: The trash cans were attracting the the flies. The trash was taken out and the flies were no longer seen in the kitchen.
Food-contact surfaces: cleaned & sanitized — Inspector Note: Observed chlorine sanitizer in ware washing machine reading below 50ppm while being used during inspection. Observed quats sanitizer in sanitizing bucket used to wipe counters at 100 ppm. CA: Chlorine concentrations shall be between 50ppm and 100ppm to properly sanitize utensils and equipment. Quaternary Ammonium concentration levels must be between 200 ppm to 400 ppm unless stated on the product that it can sanitize effectively at a different level of concentration and is EPA approved. COS: The unclean utensils were placed near 3-comp sink to be washed, rinsed, and sanitized. PIC told employees not to use ware washing machine until it is repaired. The sanitizer bucket was made again to a concentration of 200 ppm.
Proper cold holding temperatures — Inspector Note: Observed multiple TCS foods inside of the two door reach in cooler in the main kitchen cold holding at improper temperatures (see temperature log). Observed the ambient temperature for the two door reach in cooler to be above 41F. CA: Time/temperature control for safety foods must cold hold at a temperature of 41F or below. COS: The TCS foods that were at the incorrect temperatures were discarded. The cooler temperature was lowered, and any TCS foods that were at the correct temperatures were place in a working cooler or in the freezer. Discussed with PIC that the cooler temperatures should be lowered to a degree at or between 37F and 39F.
Single-use/single-service articles: properly stored, used — Inspector Note: Observed single service items (to go containers) in open boxes stored in an uninsulated area where the AC unit is located. CA: cleaned equipment and utensils, laundered linens, and single-service and single-use articles shall be stored: (i) In a clean, dry location; (ii) Where they are not exposed to splash, dust, or other contamination; and (iii) At least 6 inches (15 centimeters) above the floor. 3. Single-service and single-use articles shall be stored as specified under paragraph 1 of this subsection and shall be kept in the original protective package or stored by using other means that afford protection from contamination until used. COS: PIC discarded the to go containers on the top, and closed bag and box. PIC stated that he will move boxes into a storage area that is separate from AC unit.

Adequate handwashing facilities supplied & accessible — Inspector Note: Observed handwashing sinks next to grill and the warewashing area without paper towel or any hand drying provision. PIC stated that the shipment has not yet arrived. CA: Each handwashing sink or group of adjacent handwashing sinks shall be provided with individual, disposable towels. COS: PIC placed paper towels near each hand sink.
Food stored covered — Inspector Note: Observed Belgian waffle batter and one tub of pancake batter stored uncovered in prep top cooler by grill. The lid on one of the pancake batter bins was cracked in multiple places, leaving the batter open. Observed one metal bowl with pancake batter in it stored uncovered on counter near grill. CA: Except as specified under paragraph 2. of this subsection and when cooling as specified in DPH Rule 511-6-1-.04(6)(e)2.(ii), storing the food in packages, covered containers, or wrappings, except for loosely covered or uncovered containers in which food is being cooled if protected from overhead contamination. COS: The PIC provided lids for each item listed.
Food stored covered — Inspector Note: Observed frozen raw patties stored directly on rack in reach in freezer without any wrapping or container holding it. CA: (iv) Except as specified under paragraph 2. of this subsection and when cooling as specified in DPH Rule 511-6-1-.04(6)(e)2.(ii), storing the food in packages, covered containers, or wrappings, except for loosely covered or uncovered containers in which food is being cooled if protected from overhead contamination; COS: The bottom patty touching the rack was discarded and the rest of the patties were placed in a clean, dry container to store in the reach in freezer.
