Personal cleanliness — Inspector Note: Observed employee preparing food while wearing a bracelet. COS employee removed bracelet, washed hands, and replaced gloves. Except for a plain ring such as a wedding band food employees may not wear jewelry including medical information jewelry on their arms and hands while preparing food.

Hot and cold water available; adequate pressure — Inspector Note: Observed no water at the produce sink. Sink must be repaired; any produce requiring washing may not be used until sink is repaired.

Plumbing installed; proper backflow devices — Inspector Note: Observed leak at the 3 compartment sink.

Sewage and waste water properly disposed — Inspector Note: Observed facility dump mop bucket in the front parking lot. Condensate drainage and other non-sewage liquids and rainwater shall be drained from point of discharge to disposal according to law.

Adequate handwashing facilities supplied & accessible — Inspector Note: Upon arrival, observed paper towels stored at the top of a storage rack. Paper towels must be stored in area where they are accessible to employees. Current position allows for water from employee's hands to drip onto food or single service items on the storage rack. COS paper towels placed on counter beside hand washing sink.

Food stored covered — Inspector Note: Observed food stored uncovered inside reach in cooler. -raw salmon -raw shrimp -raw whiting -diced tomatoes -cilantro COS items were covered. Food must be stored in covered containers, or wrappings, except for loosely covered or uncovered containers in which food is being cooled if protected from overhead contamination.
