Food and nonfood-contact surfaces cleanable, properly designed, constructed, and used — Inspector Note: Observed main food warmer in front service station out of order. PIC stated that air is being release during warming process which disrupts proper warming of TCS foods. PIC has put warmer out of order and has work order. CA:Good Repair and Proper Adjustment. 1. Equipment shall be maintained in a state of repair and condition that meets the requirements specified under subsections (1) and (2)of this Rule. 2. Equipment components such as doors, seals, hinges, fasteners, and kick plates shall be kept intact, tight, and adjusted in accordance with manufacturer's specifications.

Observed attached equipment soiled with accumulated food debris. Observed food debris accumulated on kitchen floor. Observed front service drive thru wall soiled with accumulated grease. CA: Nonfood-contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residues.

Nearest sink is in the ware washing area not accessible to the front service operations. CA: A handwashing sink shall be maintained so that it is accessible at all times. for employee use.

Food-contact surfaces: cleaned & sanitized — Inspector Note: Observed staff member washing dishes and sanitizer compartment was 0ppm. CA:A quaternary ammonium compound solution shall have a minimum temperature of 75ºF (24ºC), P have a concentration as specified in DPH Rule 511-6-1-.07(6)(g) and as indicated by the manufacturer’s use directions included in the labeling, P and be used only in water with 500 ppm hardness or less or in water having a hardness no greater than specified by the EPA-registered label use instructions; COS: Water was remade to 400 ppm.

Proper cold holding temperatures — Inspector Note: Observed rice, raw chicken tenders in walk in cold held at greater than 41 degrees. PIC stated that the chicken was being used in the front service area to make orders. Rice had just been thawed in prep sink. I did not observe this during the inspection. CA: Except during preparation, cooking, or cooling, or when time is used as the public health control, time/temperature control for safety food shall be maintained at 41°F (5°C) or below.

Proper hot holding temperatures — Inspector Note: Observed mashed potatoes, dirty rice, chicken tenders being held at the front service station under 135F. CA: Except during preparation, cooking, or cooling, or when time is used as the public health control, time/temperature control for safety food shall be maintained at 41°F (5°C) or below or 135°F (57°C) or above. COS: Food was voluntarily discarded by PIC.

Warewashing facilities: installed, maintained, used; test strips — Inspector Note: Lack of use of chemical test kit when using chemical sanitizer at three-compartment sink/warewashing machine. CA: Concentration of the sanitizing solution shall be accurately determined by using a test kit or other device. COS: testing strips were provided near the 3-compartment sink. PIC has been advised to put strips in area with employee who has task of changing solution.

PIC stated that the overnight crew does not do a good enough job and they have to redo them during the morning shift.
