Personal cleanliness — Inspector Note: Observation: Observed multiple female food employees actively preparing food and handling orders with hair longer than 1/2 inch that was not properly restrained. Corrective Action: All employees were instructed that hair longer than 1/2 inch must be properly restrained to prevent hair from contacting exposed food and food-contact surfaces. Employees were instructed to use an approved hair restraint, such as a hairnet or other effective restraint, as required by the Food Code. Employees were also instructed to properly restrain facial hair longer than 1/2 inch. Applicable Requirement – Hair Restraints: Employees preparing or handling food shall use effective and clean hair restraints, such as hairnets or other approved restraints, worn properly to restrain loose hair, including beards and mustaches longer than 1/2 inch.

Personal cleanliness — Inspector Note: Observed food employee wearing jewelry other than a plain ring on their hands/arms while preparing food. Observation: Three food employees were observed wearing jewelry other than a plain ring on their hands/arms while preparing food. Specifically, one food handler was wearing bracelets and rings, two other employees were wearing bracelets. CA: Except for a plain ring, such as a wedding band, food employees may not wear jewelry (including medical information jewelry) on their hands and arms while preparing food. Was not Corrected on site: Employees were instructed to remove prohibited jewelry before continuing food preparation. Employees did not remove jewelry

Adequate ventilation and lighting; designated areas used — Inspector Note: Observed an employee's plastic water bottle stored on counter in proximity to clean equipment and utensils. The employee was instructed to immediately remove the water bottle from the counter. Employees were reminded that personal items must be stored in a designated area away from food, food-contact surfaces, clean equipment, utensils, and single-service/single-use articles to prevent potential contamination.

Insects, rodents, and animals not present — Inspector Note: Observation: Observed live flies in kitchen. Observed live flies in the kitchen and food preparation area where exposed food was present. The Person in Charge was notified and instructed to immediately eliminate the flying insect activity and take measures to prevent further entry and harborage. Management was instructed to maintain drive thru doors and openings closed or adequately protected by not leaving open for prolonged periods of time, eliminate conditions that may attract or harbor flies, and implement appropriate pest-control measures. he establishment shall be maintained free of insects and other pests. Effective measures shall be used to prevent the entrance and presence of pests and to protect food and food-contact surfaces from contamination.
