Nonfood-contact surfaces clean — Inspector Note: Shelves unclean in the walk in cooler, exterior of equipment unclean with heavy build-up, interior and exterior of the burger freezer is unclean with debris and counters in corners. Observed gaskets of the reach in freezer to be unclean with food debris. CA: Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris.

Physical facilities installed, maintained, and clean — Inspector Note: Observed stagnant water/ holding on the floor tiles, outside the entrance door tiles, in front of the walk in cooler and freezer and some by the drive through line (next to the bread freezer). CA: Informed person in charge that the coved of the floor tiles will need to be replaced or resurface to prevent it from holding water. The floors in food service establishments in which water flush cleaning methods are used shall be provided with drains and be graded to drain, and the floor and wall junctures shall be coved and sealed

The same for the back door (by the computer desk) CA: outer openings of a food service establishment shall be protected against the entry of insects and rodents by: Filling or closing holes and other gaps along floors, walls, and ceilings; Closed, tight-fitting windows; and Solid, self-closing, tight-fitting doors.

Toxic substances properly identified, stored, used — Inspector Note: Observed QUAT sanitizer bucket reading at 400ppm+. CA: Chemical sanitizers, including chemical sanitizing solutions generated on-site, and other chemical antimicrobials applied to food-contact surfaces shall: Meet requirements for Food-contact surface sanitizing solutions. for QUAT it should be around 200-300ppm. COS: sanitizer bucket was re-made reading at 200ppm.
