Observed rice cooker in the back of the kitchen with a build up of food debris. CA: The food-contact surfaces of cooking equipment and pans shall be kept free of encrusted grease deposits and other soil accumulations. Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris. Correct by: Cleaning and sanitizing fryer baskets and rice cooker. This equipment should be cleaned on a regular basis to avoid build up in the future.
Certified Food Protection Manager — Inspector Note: Observed CSFM displayed expired(10/30/2025). CA: A CFSM certificate which has expired, been revoked or suspended shall not be posted in the food service establishment. 3. All licenses, certificates, diplomas, or other similar credentials issued or granted to an owner or operator who has successfully completed an approved or accredited food safety certification course and exam shall expire on the expiration date determined by the credentialing organization. Within ninety days of the expiration of the CFSM certificate, the CFSM shall enroll in an approved food safety training course, pass an approved exam and obtain a new certificate. Correct by: Informing inspector when a new CSFM certificate has been obtained by emailing a copy to [REDACTED EMAIL].
Food separated and protected — Inspector Note: Observed food stored incorrectly in the walk-in cooler. Observed raw shrimp, scallops, and fish stored above RTE foods(egg rolls, pasta). Observed a bag of imitation crab (fully cooked) thawing in a bowl with raw scallops. CA: Food shall be protected from cross contamination by separating raw animal foods during storage, preparation, holding, and display from: (I) Raw ready-to-eat food including other raw animal food such as fish for sushi or molluscan shellfish, or other raw ready-to-eat food such as fruits and vegetables, and (II) Cooked ready-to-eat food; and (III) Fruits and vegetables before they are washed; (IV) Frozen, commercially processed and packaged raw animal food may be stored or displayed with or above frozen, commercially processed and packaged, ready-to-eat food. COS: PIC moved raw items to shelf below RTE foods, and removed imitation crab from bowl with raw scallops. Provided PIC with stacking handout to be displayed in the cooler.

Observed raw chicken removed from its original packaging stored above frozen egg rolls and other RTE foods. CA: Food shall be protected from cross contamination by separating raw animal foods during storage, preparation, holding, and display from: (I) Raw ready-to-eat food including other raw animal food such as fish for sushi or molluscan shellfish, or other raw ready-to-eat food such as fruits and vegetables, and (II) Cooked ready-to-eat food; and (III) Fruits and vegetables before they are washed; (IV) Frozen, commercially processed and packaged raw animal food may be stored or displayed with or above frozen, commercially processed and packaged, ready-to-eat food. Correct by: Arranging storage in the walk-in freezer to be compliant by not storing any raw meats removed from commercial packaging above ready to eat foods. Provided PIC with stacking hand out.
