CA: Except for a plain ring such as a wedding band food employees may not wear jewelry including medical information jewelry on their arms and hands while preparing food. COS: Ring was removed.

Insects, rodents, and animals not present — Inspector Note: Observed live roach in the kitchen area (by the dry food storage area). CA: he presence of insects, rodents, and other pests shall be controlled to minimize their presence on the premises by: Routinely inspecting incoming shipments of food and supplies; Routinely inspecting the premises for evidence of pests.

Toxic substances properly identified, stored, used — Inspector Note: Observed bleach bucket sanitizer to be reading high at 200PM++. CA: Chemical sanitizers, including chemical sanitizing solutions generated on-site, and other chemical antimicrobials applied to food-contact surfaces shall: Meet requirements for active and inert ingredients for use in antimicrobial formulations (Food-contact surface sanitizing solutions). COS: Sanitizer solution was re-made to 100ppm.
