CA: Nonfood-contact surfaces of equipment that are exposed to splash, spillage, or other food soiling or that require frequent cleaning shall be constructed of a corrosion-resistant, nonabsorbent, and smooth material. Discussed with employee to remove all non-approved materials lining shelves. If wanting to line shelves plastic or rubber mats that meet the requirements are allowed.

COS: item removed CA: A handwashing sink shall be maintained so that it is accessible at all times for employee use. 2. A handwashing facility may not be used for purposes other than handwashing.
Food-contact surfaces: cleaned & sanitized — Inspector Note: Observed dish machine reading 10ppm cl. After several attempts still reading 10ppm cl. Discussed manually washing all dishes at 3-compartment sink until dish machine reads 50-100ppm according to the manufacturer's directions. CA: A chemical sanitizer used in a sanitizing solution for a manual or mechanical operation at contact times specified under subsection (8)(b)3 of this Rule shall meet the requirements specified in DPH Rule 511-6-1-.07(6)(g), shall be used in accordance with the Environmental Protection Agency (EPA)-registered label use instructions

Physical facilities installed, maintained, and clean — Inspector Note: Observed non-food contact surfaces of walls unclean with grease and food debris build-up. Discussed with PIC to clean areas and implement a regular cleaning scheduling to prevent excess build-up overtime. CA: The physical facilities shall be cleaned as often as necessary to keep them clean and by methods that prevent contamination of food products.
Food separated and protected — Inspector Note: In walk in cooler, observed raw shelled eggs stored above vegetables tamales. COS: Vegetable tamales and raw shelled eggs relocated so that the tamales are stored above the raw eggs. CA: Food shall be protected from cross contamination by: (i) Except as specified in paragraph (i)(IV) of this subsection, separating raw animal foods during storage, preparation, holding, and display from: (I) Raw ready-to-eat food including other raw animal food such as fish for sushi or molluscan shellfish, or other raw ready-to-eat food such as fruits and vegetables, P and (II) Cooked ready-to-eat food;
Food separated and protected — Inspector Note: Observed food employee wash gloved hands at hand sink. Inspector instructed employee to remove gloves then wash hands then put on new gloves. COS: employee removed gloves and washed hands before putting on new gloves. CA: If used, single-use gloves shall be used for only one task such as working with ready-to-eat food or with raw animal food, used for no other purpose, and discarded when damaged or soiled, or when interruptions occur in the operation.
Food stored covered — Inspector Note: Observed 2 containers of plantain chips and 1 container of peeled onions stored uncovered. COS: foods covered with saran wrap. CA: Except as specified under paragraph 2. of this subsection and when cooling as specified in DPH Rule 511-6-1-.04(6)(e)2.(ii), storing the food in packages, covered containers, or wrappings, except for loosely covered or uncovered containers in which food is being cooled if protected from overhead contamination;
