Physical facilities installed, maintained, and clean — Inspector Note: Floors, Walls, and Ceilings, Cleanability. Except as specified under subsection (2)(d) of this Rule and except for anti-slip floor coverings or applications that may be used for safety reasons, floors, floor coverings, walls, wall coverings, and ceilings shall be designed, constructed, and installed so they are smooth and easily cleanable. At the time of inspection, area under deep fryers were observed with a heavy accumulation of grease residue and hole in floor observed next to prep top cooler. Person in Charge was advised to have floors cleaned and repaired.

Adequate handwashing facilities supplied & accessible — Inspector Note: Observed hand-sink being turned on during the beginning of inspection, food was prepped and being hot held. CA : handwashing sink shall be maintained so that it is accessible at all times for employee use COS : PIC turned on hand sink prior to washing and instructed employees to wash their hands.
