Personal cleanliness — Inspector Note: Observed chef cooking at the grill in the main kitchen with a beard longer than a 1/2 inch and not wearing a beard guard. CA: Employees preparing or handling food shall use effective and clean, disposable or easily cleanable nets or other hair restraints approved by the Health Authority, worn properly to restrain loose hair including beards and mustaches longer than one half inch.

Utensils, equipment and linens: properly stored, dried, handled — Inspector Note: Observed several stacks of metal pans stacked while wet on the dish rack. CA: Advised PIC After cleaning and sanitizing, equipment and utensils shall be air-dried or used after adequate draining before contact with food; and may not be cloth dried except that utensils that have been air-dried may be polished with cloths that are maintained clean and dry.

Nonfood-contact surfaces clean — Inspector Note: Observed food debris and buildup on the floors of the walk-in cooler. CA: Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris.

Adequate ventilation and lighting; designated areas used — Inspector Note: Observed an employee's electric scooter stored in the main kitchen. CA: Lockers or other suitable facilities shall be provided and used for the orderly storage of employees' clothing and other possessions. COS: Food employee moved the electric scooter to the designated employee area.

Food stored covered — Inspector Note: Observed a food container of cooked mushrooms stored under the prep top cooler without any lid or covering. CA: Food shall be stored in packages, covered containers, or wrappings, except for loosely covered or uncovered containers in which food is being cooled, to be protected from overhead contamination. COS: Food employee provided a lid for the food container of cooked mushroom in the prep top cooler
