Contamination prevented during food preparation, storage, display — Inspector Note: Observed bag of carrots in walk in cooler with black organic and white organic substance stored on the shelf. Owner stated they forgot to inspect the delivery. CA: Food shall be protected from contamination that may result from a factor or source. COS: Bag was voluntarily discarded.

Observed the sealing of the deep freezer near the back door unclean with a red substance accumulated. Observed bottom of reach in cooler unclean to sight and touch with black organic substance build up. CA: Nonfood-contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residues.

Physical facilities installed, maintained, and clean — Inspector Note: Observed food debris accumulated on kitchen floor. Observed wall soiled with accumulated grease. Observed grease accumulated under cooking equipment. Discussed with PIC that deep cleaning shall be done by correct date. CA: The physical facilities shall be cleaned as often as necessary to keep them clean and by methods that prevent contamination of food products. Except for cleaning that is necessary due to a spill or other accident, cleaning shall be done during periods when the least amount of food is exposed, such as after closing.
