Adequate ventilation and lighting; designated areas used — Inspector Note: Observed failure to have adequate lighting in food storage areas. Observed a dim and flickering light bulb in walk in freezer. (f) Lighting Intensity. The light intensity shall be: 1. At least 10 foot candles (108 lux) at a distance of 30 inches (75 cm) above the floor, in walk-in refrigeration units and dry food storage areas and in other areas and rooms during periods of cleaning; PIC advised to change the lightbulb in walk in freezer.

Except where a toilet room is located outside a food service establishment and does not open directly into the food service establishment, such as a toilet room that is provided by the management of a shopping mall, a toilet room located on the premises shall be completely enclosed and provided with a tight-fitting and self-closing door.

Insects, rodents, and animals not present — Inspector Note: Observed a live roach in the kitchen area by the reach-in cooler. 511-6-1.07(5)(l) - Removing Dead or Trapped Birds, Insects, Rodents, and Other Pest (C) (l) Removing Dead or Trapped Birds, Insects, Rodents, and Other Pests. Dead or trapped birds, insects, rodents, and other pests shall be removed from control devices and the premises at a frequency that prevents their accumulation, decomposition, or the attraction of pests.

Adequate handwashing facilities supplied & accessible — Inspector Note: Observed handwash sink not accessible for employee use at all times. The only handwash sink in the kitchen was blocked by containers. The sink was totally dry indicating no employee recently used it. although kitchen was actively cooking and serving food. 511-6-1.06(2)(g) - Handwashing Sinks, Nos/Capacities (Pf) (g) Handwashing Sinks, Numbers and Capacities: A food service establishment shall provide, a number of handwashing sinks necessary for convenient use by employees in areas specified under subsection (2)(l) of this Rule, and not fewer than one handwashing sink or fewer number of handwashing sinks required by law. PIC instructed not to block the handwash sink so that it can always be accessible.

Food-contact surfaces: cleaned & sanitized — Inspector Note: Observed pink and black mold-like substance on the interior flap of the ice machine. 511-6-1.05(7)(a)1 -Equipment food-contact surfaces and utensils shall be clean to sight and touch. Instructed PIC ice machine shall be cleaned regularly to prevent buildup.
