Single-use/single-service articles: properly stored, used — Inspector Note: Observed plastic single service bowls being used as a scoop in corn starch container and cooked white rice container. Single service items can not be reused and all scoops must have a handle. PIC removed bowls.

Warewashing facilities: installed, maintained, used; test strips — Inspector Note: Observed no chemical test strips for sanitizer at 3 compartment sink. CA: Advised PIC to obtain chlorine test strips. Concentration of the sanitizing solution shall be accurately determined by using a test kit or other device. Pf.

Nonfood-contact surfaces clean — Inspector Note: Observed a build-up of grease on grill hood in main kitchen area. Observed build-up of grease in between equipment around grill area. Observed soiled residue inside of microwave. Observed soiled residue on the outside of equipment throughout main kitchen. CA: Advised PIC to routinely clean all non-food contact surfaces. Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris.

Physical facilities installed, maintained, and clean — Inspector Note: Observed broken floor tiles throughout main kitchen area. Observed grill hood in despair/poor condition. CA: Advised PIC to have tiles replaced. Advised to have grill hood replaced. Floors, Walls, and Ceilings, Cleanability. Except as specified under subsection (2)(d) of this Rule and except for antislip floor coverings or applications that may be used for safety reasons, floors, floor coverings, walls, wall coverings, and ceilings shall be designed, constructed, and installed so they are smooth and easily cleanable.

Food stored covered — Inspector Note: Observed flour stored uncovered in pantry area. Observed RTE wonton chips stored uncovered on rack in main kitchen. Observed cooked beef stored uncovered in reach-in cooler. COS: PIC covered items.
