Single-use/single-service articles: properly stored, used — Inspector Note: Observed boxes of foods stored on walk-in freezer floor. 511-6-1.05(10)(e)1&3 - Single-Service/Single-Use items stored 6" off floor in clean, dry location (C) (e) Equipment, Utensils, Linens, and Single-Service and Single-Use Articles. 1. Except as specified in paragraph 4 of this subsection, cleaned equipment and utensils, laundered linens, and single-service and single-use articles shall be stored: (i) In a clean, dry location; (ii) Where they are not exposed to splash, dust, or other contamination; and (iii) At least 6 inches (15 centimeters) above the floor. PIC moved boxes off the floor.

Physical facilities installed, maintained, and clean — Inspector Note: Observed broken tile underneath the hand sink. observed failure to provide smooth, nonporous, and easily cleanable ceiling tiles in the dry storage room across from the kitchen and above the reach-in freezer in the main kitchen (2 ceiling tiles). PIC was advised that the ceiling tiles must be smooth, durable, and easily cleanable.

PIC instructed that all ready-to-eat, potentially hazardous food held more than 24 hours must have a prepared or discard date.
